Thanks for your interest in TLCS 2006. We look forward to receiving your application.

This application will be used both to help determine if your abilities and experience
are sufficient for you to safely participate in TLCS, and to help us design the course
to meet your needs should you participate.

Please keep in mind that your complete honesty is necessary for the sake of your
own safety as well as the safety of those with whom you may climb during TLCS.
It is extremely important that you DO NOT misrepresent your experience or abilities.

This document is formatted as a simple text document and can be completed with a
simple text editor, such as Notepad. We ask that you tab your responses for readability.
